Please fill these out before your first Bodywork/Massage session with me. If you don't have a printer, I can provide the first 3 forms and fill them out with you at my home before your session. Bodywork and Massage beyond Intake forms One and Two, and Massage Client Waiver Form
The advanced work below for customers is optional, and must be applied for in writing.
As of January 2015, The Massage Therapy Act of California was created and enforceable to protect clients and massage therapists who are CAMTC Certificate holders. Included in the Massage Therapy Act is that exposure of or working on the breast area is now not allowed without written permission first from the client, and then a referral from a Licensed California Health Care Provider. This may be an MD, Marriage and Family Counsoler, Registered Nurse, Chiropractor, Acupuncturist, or a Physical Therapist. I created the Consent and Referral form below to make it easy. Just hand it to your health care provider, that includes my CAMTC license number, and have them sign and date it. Before you go, we can both sign and date this document together after talking about the benefits and agreed conditions for your session in this area. This new CAMTC law is good, and I appreciate the women that have taken the effort to protect me as a Massage Therapist / Bodyworker while I do chest and breast area work, lymphatic chest drains, and other postural work on them. This work is important for arm range of motion, shoulder work, and clearing lymphatic congestion. See anatomy pictures posted below.


Women who would rather not have work in the chest or breast area can wear a bra, and will also be draped with a towel or sheet during their session. It is common to work on a woman at least 5 times before considering the topic of work in the chest area. Some clients have waited a year before receiving this kind of lymphatic massage therapy. Please understand that you will always be welcome for sessions without work in this area.
